This contribution addresses the theme of the digitalization of healthcare documentation, specifically focusing on the study of a sample of medical records (1996 - 2022) from the Regional Center for Neurogenetics (CRN) in Lamezia Terme, centered on neurodegenerative diseases and characterized by a high degree of documentary heterogeneity. Beginning with a conceptualization of the medical record and an overview of the main reflections highlighted in the relevant state of the art regarding its relationship to the health record, the paper presents the principal challenges associated with defining the medical record, particularly those linked to its structural evolution. From a methodological standpoint, it describes the process of knowledge extraction from digitized documentation through semantic annotation techniques, which are useful for the identification and automatic encoding of relevant clinical information (e.g., diagnoses, treatments). The aim, through the adoption of standards such as XML, is to ensure improved formalization, structuring and metadata enrichment of documents, thereby guaranteeing data interoperability and uniformity. In this way, the process of preserving the clinical and scientific memory of the CRN is facilitated, ensuring an adequate level of accuracy in clinical decision-making for the management of neurodegenerative diseases.

Abstract
In this way, the process of preserving the clinical and scientific memory of the CRN is facilitated, ensuring an adequate level of accuracy in clinical decision-making for the management of neurodegenerative diseases.I documenti in IRIS sono protetti da copyright e tutti i diritti sono riservati, salvo diversa indicazione.
