Traumatic injuries of the occipito-cervical junction represent a clinical entity more and more frequent. Conservative or surgical treatment of atlo-occipital instability is still controversial; the choice depends mainly on the alteration of articular biomechanic caused by the trauma. Other parameters are however to be taken into account (the patient's age, general condition etc.). We report a critical review of our experience concerning 79 patients managed, between 1987 and 1997, conservatively by external fixation devices or surgically through decompression and/or internal stabilization with metallic instruments. We compare our experience with that of other authors and the results we have got are reported as well.
